Overview: Full Time Physical Therapist needed for Outpatient Clinic! $7,500 Sign on Bonus 40 hours/week vary for clinic hours between 7a-6pm M-F with some weekend rotations Seeking a PT with Pelvic Floor Therapy experience! Under the general supervision of the Manager of Rehabilitation Services, functions as a member of the Physical Therapy team; evaluates and establishes treatment programs for variety of patient diagnosis; treats patients in a variety of settings; provides patient/family education; performs necessary documentation; and supervises supportive personnel.
COVID-19 and Flu Vaccination Requirement: It is required to be vaccinated for COVID-19 and Influenza.

org Responsibilities: TECHNICAL/ADMINISTRATIVE/CLINICAL: (Total Weight of 100) Demonstrates competence in the skills, (processes, procedures, and equipment) necessary to carry out assigned duties as identified below.
Demonstrated competence in recognizing and responding to patients physical, mental, emotional and developmental needs should be evaluated in relation to the patient populations served as appropriate for each responsibility.
Evaluates and plans treatment programs demonstrating competence in the administration of age appropriate care, for a variety of patient diagnoses and variety of settings, which may include but is not limited to the following: Homecare, TCU, Rehab, General Acute.
, Outpatient, and Outside Agency.
Provides age appropriate care in accordance with established standards of care, policies, procedures, and state and federal law and summarizes treatment plan.
Completes all documentation in a timely, effective manner, communicating with physicians, other Trinity Medical Center departments, and outside agencies as needed to enhance patient care and determine short and long term goals.
All documentation will be timely and efficient according to Rehabilitation documentation policy.
Supervises supportive personnel in immediate work area.
Supervises Physical Therapy and Physical Therapist Assistant students as assigned by CCCE.
Attends and participates on active level in staff meetings.
Attends continuing education seminars and conducts appropriate informative in-services programs to staff.
Presents literature review and equipment in services to staff on rotating basis.
Assist with marketing of outpatient clinics to physicians as requested by Manager.
Qualifications: EDUCATION, TRAINING & EXPERIENCE: Bachelor's degree, Certificate or entry level Master's degree or Doctorate from an accredited Physical Therapy Program.
KNOWLEDGE, ABILITIES & SKILLS: Must be able to safely lift patients and operate various physical therapy equipment.
Must be able to evaluate, plan treatment programs, and treat patients effectively.
Must possess interpersonal skills necessary to deal effectively with patients, physicians, other Trinity employees, outside agencies, and community.
LICENSES, REGISTRATIONS AND CERTIFICATIONS: Current IL and/or IA state license.
